FUE and DHI share the same extraction step; they differ in how grafts are placed. In Sapphire FUE, channels are opened first with a sapphire crystal blade and grafts are then placed into them, which gives the surgeon maximum control over angle and direction at the hairline. In DHI, a Choi implanter pen opens and fills the channel in one movement, which shortens the time grafts spend outside the body and allows work between existing hairs — the reason DHI dominates crown density and unshaven cases. Neither is universally superior. At Sule Hair Clinic most patients receive both in a single Hybrid session: sapphire at the hairline, DHI in the crown.

Healing is similar; both crust between day 4 and day 10 and shed transplanted hairs in week 2–4 before regrowth begins around month 3–4.
This is the real clinical difference. In sapphire FUE the channels are opened first and the grafts wait in a holding solution before placement; in DHI each graft goes from forceps to Choi pen to scalp in one movement, so the out-of-body time is shorter. Both are safe when cooling and solution protocols are followed.
