A second session is common in two situations: hair loss that continued after a first procedure, and plans that were staged from the beginning because the area involved was larger than one session could cover well. The deciding factor is a measured donor assessment — donor density per square centimetre, scalp laxity, hair calibre and how the first session was harvested — not how much coverage you would like to add.
Ten to twelve months is the normal minimum. Two separate processes need that time. The first result is not final until around month twelve, so planning earlier means planning against an unfinished picture and risking grafts placed where nature was about to fill in anyway. Separately, the donor area needs months to revascularise properly after extraction; harvesting again too early lowers graft quality and can leave the donor looking thinner than it needed to.
A typical scalp donor area can safely provide roughly 6,000–7,000 grafts across a lifetime, and some patients considerably fewer. That figure depends on donor density, hair calibre, scalp laxity and how conservatively earlier sessions were harvested. It is the single most important number in second-session planning, because follicles that are removed do not regenerate — the donor is a finite budget, not a renewable supply.
Advanced loss covers a large surface. Transplanting all of it in one day would exceed what the donor can safely give and what the scalp can heal well, and spreading a fixed graft number across too much area produces even but see-through coverage everywhere. Staging solves both problems: the frontal third and mid-scalp are built to a density that frames the face, then the crown is completed in a second session once the donor has recovered and the first result is mature.

A repair procedure corrects a problem — a hairline drawn too low or too straight, grafts placed at the wrong angle, clumped multi-hair grafts at the front line, or an over-harvested donor area. Repair work is finer, needs more single-hair grafts and often more grafts in total, while the donor available is already reduced.
The skin surface closes within one to two weeks, but that is not the same as recovery. Full revascularisation and tissue settling take months, and the shocked hairs that were not extracted need three to six months to regrow before remaining density can be measured honestly. A proper second-session assessment re-measures donor density zone by zone and compares it against the map from the first procedure.
