On your back, with your head raised at roughly 45 degrees, using a travel-style neck pillow so the back of your head rests on the pillow's ring and the grafted area touches nothing. Our clinic's instruction is to keep this position for the first 10 nights. Two pillows behind the shoulders, a wedge cushion or a reclining chair all work; what matters is that the head stays elevated and does not roll onto the recipient area.
Ten nights is the instruction we give every patient, and the neck pillow is provided in the post-operative kit. It exists to stop your head rolling onto the grafts while you are asleep, which is the part of the night nobody can control consciously. After the tenth night the grafts are secure enough that a normal pillow can be used, and most patients keep the neck pillow a few nights longer simply because the donor area still feels tight.
From around night eleven, and gradually. Start with a normal pillow while still lying on your back, then move to your side once you can do so without the transplanted area pressing into the pillow. If your grafts are at the crown or the sides, give it a few more nights, because those areas make contact more easily than a front hairline. Face-down sleeping is the last to come back and should wait until the area is no longer tender.
Two reasons. Elevation limits the fluid that gathers in the forehead and around the eyes during the first days, which is the swelling most patients notice on day two or three. It also keeps your head from turning face-down or pressing sideways into the mattress, which is how newly placed grafts get rubbed. The angle is not about comfort — it is the simplest way to protect grafts during the hours you are not awake to protect them yourself.
Light contact with the back of the head is unavoidable and is not a problem for the grafts themselves, which sit in the recipient area. The donor area is a healing wound surface, though, so it often feels tight or tender for the first week and direct pressure on it is uncomfortable. The neck pillow solves both issues at once: the ring supports your neck while the back of the head rests inside the opening rather than on a flat surface.
Brief accidental contact in the first nights is common and rarely does damage on its own; grafts become progressively more secure over the first ten days. What causes real problems is repeated rubbing — sleeping face-down, or a head that rolls onto the same spot for hours. If you wake up having slept on the area, do not panic and do not touch it to check: look in a mirror, photograph anything that looks displaced or is bleeding, and send it to your post-operative team the same day.
