Every surgical procedure leaves some mark, so the honest answer is yes — but the type matters. Modern FUE removes follicles one by one through punches under a millimetre wide, leaving small round marks in the donor area rather than a line. FUT, the older strip method, leaves a single linear scar across the back of the head. In both cases the marks sit in the donor area and are covered by hair at normal lengths.
The tiny extraction points themselves are permanent as skin marks, but they become progressively less noticeable. They are visible as small pink dots in the first weeks, fade to skin tone over roughly two to six months depending on skin type, and are then hidden by donor hair once it grows back to a normal length. What makes them stand out long term is not the technique itself but oversized punches, extractions concentrated in one spot, or poor wound care.
FUE leaves scattered round marks under a millimetre across, spread over a wide donor surface — compatible with short haircuts for most patients. FUT leaves one horizontal linear scar where a strip of scalp was removed; it hides well under medium-length hair but shows if the head is shaved. FUE is the standard approach in our clinic; both Sapphire and DHI are FUE-based extraction methods.
At a number 1 or 2 clipper cut, well-executed FUE marks are usually hard to see from conversational distance, though they can be noticeable under direct light or a camera flash. Fully shaved with a razor, some scattered dots are typically visible on close inspection. An FUT linear scar will show at any shaved length. If shaving your head is part of your look, say so during your consultation so extraction is planned accordingly.
Often yes. Grafts can be placed into scars from accidents, burns, surgery or an old FUT strip, and doing so is one of the most common repair requests we see. Scar tissue has a poorer blood supply than healthy scalp, so density is planned conservatively and the work is sometimes staged across more than one session. Suitability depends on how mature, soft and vascular the scar is, and is assessed with photographs and an examination first.
Large or blunt punches, taking too many grafts from too small an area, extractions outside the safe donor zone, smoking during healing, picking or scratching scabs, sun exposure on fresh marks, and an individual tendency to keloid or hypertrophic scarring. Most of these are decisions made in the operating room rather than traits of the patient, which is why donor planning and who performs the procedure matter more than the brand name of a technique.
