Dead hair follicles appear as smooth, shiny bald skin with no visible pores, no fine baby hairs (vellus), and no dark dots. Under dermoscopy they show a loss of follicular openings — the tiny holes on the scalp are gone. This shiny, poreless texture is the clearest sign that a follicle is no longer viable.
Dormant follicles are miniaturized but still alive: you'll see thin, colorless vellus hairs, visible pores under close inspection, and often greasy or itchy skin. Dead follicles show no hair, no pores, and shiny, tight skin. A trichoscopy exam and pull test at a hair clinic can confirm which category your scalp falls into.
Truly dead (fibrosed) follicles cannot be revived — the stem cells are gone.
