Before month twelve almost nothing can be judged. Transplanted hairs shed in the first weeks, regrowth starts around month three to four, and density keeps building until month twelve to eighteen. A genuinely poor result is not about thin coverage at month four; it is about design and placement you can see in a photograph at any stage: a hairline drawn too low or in a straight line, hair emerging in the wrong direction, visible clumps instead of single hairs at the front, gaps in a pattern that has no logic, or a donor area that has been thinned in one concentrated patch.
Usually the opposite. Transplanted hairs commonly shed between week two and week six while the follicle stays in place and restarts its cycle — this is shock loss, and it is an expected part of healing rather than a lost graft. Existing native hairs around the implanted area can shed as well and normally return within three to six months. Shedding becomes a question worth asking when it continues past month six with no regrowth in the same zone.
Repair work is normally considered from twelve to eighteen months after the original procedure. Waiting is not delay for its own sake: the final density has to be visible before anything is added, scalp tissue needs to have settled, and any marks in the donor area have to mature before further extraction is planned. The exception is a graft placed in a direction or position that will never look natural — that can sometimes be addressed earlier, because time will not improve it.
